P. 14 · BRIEF

NAC.

Glutathione precursor. Better evidence than the parent molecule.

N-acetylcysteine is the most efficient route to raising endogenous glutathione, and a well-studied molecule in its own right. Modest acne and inflammation data; mood and trichotillomania endpoints; safer profile than IV glutathione at a fraction of the price.

What the literature shows.

Inflammatory acne adjunct
Small RCTs, 600 mg × 2/d

Modest reduction in lesion count over 12 weeks. Useful adjunct alongside topicals.

55%
Trichotillomania
Established trials

Off-label but well-supported indication. Reduces hair-pulling urges over 12 weeks.

75%
Glutathione biomarkers
Replicated

The most efficient route to raising endogenous glutathione status. The science here is solid.

85%
PCOS-related skin presentations
Small trials

Useful in PCOS-driven acne and hirsutism, often in combination with myo-inositol.

Forms and bioavailability.

NAC 600 mg capsule

Absorption · Good

Standard form. Twice daily on an empty stomach for skin endpoints.

NAC 1200 mg sustained-release

Absorption · Good

Once-daily convenience. Equivalent endpoints in extended-release trials.

Effervescent NAC

Absorption · Good

Used in respiratory indications. Sulfur taste is the limiter; capsules are easier.

Bottom line

A useful, defensible, inexpensive supplement with a respectable evidence base. The smart pivot from oral glutathione for anyone interested in the antioxidant pathway.

Frequently asked.

Is it safer than IV glutathione?

Substantially. NAC has been used clinically for decades (paracetamol overdose, mucolytic in respiratory disease) at doses higher than the skin protocol. The safety profile is well-mapped, the cost is low, and the gut-bypass concerns of oral glutathione are sidestepped via the precursor route.

How long for skin effects?

8–12 weeks for inflammatory acne adjunct work. The glutathione biomarker shift is faster (4–6 weeks), but skin endpoints lag the biochemistry.

Will it interact with anything?

NAC reduces nitroglycerin tolerance and may interact with anticoagulants at high doses. Most other interactions are minor. Discuss with your clinician if you take cardiovascular medication.

What about mental health claims?

There is genuine, replicated literature on NAC in OCD-spectrum conditions, trichotillomania, and depression as adjunct. The skin-mood connection is real for inflammatory acne; whether NAC drives benefit through skin or mood pathways is unclear.
